My kappa and lambda free light chain numbers are both in the normal range. However, because the kappa number is at the high end of normal compared to my lambda number, the ratio is out of range. Is this a concern for SMM? Does this suggest a quicker progression to active multiple myeloma?
Thanks.
K/L ratio: 2.07
Kappa FLC: 18.8 mg/L
Lambda FLC: 9.1 mg/L

Re: Should I be concerned about my kappa / lambda FLC ratio?
Hi FingersCrossed,
The kappa/lambda ratio that you have is not that high at all. In the Mayo system for classifying smoldering myeloma into different risk-of-progression categories, the ratio has to be either above 8, or below 0.125, for it to be considered a risk of earlier progression to symptomatic myeloma.
See the discussion in this thread for summaries of the Mayo Clinic and Spanish smoldering myeloma risk classification systems and, if you want to learn even more, read the ASH Education Program paper that is mentioned in Ricardo's posting in the thread.

Re: Should I be concerned about my kappa / lambda FLC ratio?
Thanks, Cheryl.
Just took a look at the article!
I'm still trying to digest/process/understand all of the numbers. I have a lot of questions for the specialist I'm going to see soon, but I've been able to alleviate some of the anxiety I've had since diagnosis by educating myself about some of the more important disease markers.
I think the last hurdle is to get a good understanding of the data from my FISH test.
