hI Coop, That's great that you are able to compare your test results from when you were diagnosed. Sounds like great strides have been made in treating your myeloma. I find that reviewing my test results helps me when I am talking with my myeloma oncologist too...gives me a reference point and helps in the understanding of it too.

My Grandad (70 yrs. old when diagnosed) passed away recently ,fighting multiple myeloma.
The end was predicted on the day, a few hours earlier to death. He was leading a pretty normal life and went with us on a trip few days earlier. 7 days ago, he fell in the toilet and was taken to hospital. He was kept under oxygen mask for his entire time there.
As i was saying, few hours earlier to death the doc came about and examined him.
He predicted his last stanza-
At this rate,platelets will be produced less and less.
All in all blood won't reach all organs.
So he'll feel breathless leading to high pulse rate(about 200).
Eventually trachypnia(very low pulse rate) will happen and he'll stop breathing...
Exactly how it happened.
He died at age 72 or 73.(not sure, still in shock)

Dear 'God Be With You', I am really sorry to hear about your grandfather's passing. At least he was leading an active life up until he was hospitalized, and he had his family nearby. You have my deepest sympathies. Nancy

Here are some things I have learned from my reading about the serum free light chain test.
1.each plasma cell produces just one type of immunoglobulin (Ig)
2.When you have multiple myeloma, just that one type of Ig is cancerous, that one type of Ig is produced in excess and it is called a monoclonal protein - the M protein or M spike
3. each Ig is made of light chains & heavy chains. There are 5 basic heavy chains - IgG, IgA, IgM, IgD & IgE
4. there are 2 kinds of light chains - Kappa and Lambda.
5. The immunoglobulins (Ig) are produced and assembled in the plasma
6. Unattached light chains are called free light chains. It is typical to produce more light chains that needed - MGUS if often detected & monitored by the light chains
7. The most sensitive tests for light chains is an assay of the blood serum. This is more sensitive than testing urine (UPEP) or blood (SPEP)
8. Normal :
Kappa 3.3-19.4
Lambda 5.71-26.3
Kappa/Lambda ration .25/1.65
if impaired renal .37- 3.1
If the ratio is abnormal, the myeloma is active
If a treatment works the myeloma cells will die and the light chain levels will decrease within hours.
My Stem Cell replacement was 11/19/2012 and I did not go into remission - I already have abnormal ratios and increasing numbers of kappa light chains. Now, I am eager to find out more about exact numbers.

I was diagnosed with multiple myeloma in on July 23, 3013 with a count of 88,000 of lambda . As of 8/29 my numbers have dropped to 74.2 because of the Velcade. Hopefully, overtime the Velcade will bring your numbers down too. Because of the drop I am now a candidate for a bone marrow transplant. YES!

kappa free light chain results
I'm new here. My Dr. says I don't have a full blown case of Multiple Myeloma, but he's giving me Revlimid to halt the progress of my Myelodysplasia. My latest readings are:
Kappa light chain, free 2850.0 H
Lambda " " " 14.6
Free, K/L ratio 195.21 H
When I started on Revlimit last spring, the Kappa was up around 3450.
