Just Diagnosed with MGUS. Lab help please

Hi all, well I finally got my results from the Hematologist. He said I had MGUS. I was a little confused though with the lab results he gave me. The flags on my labs were:

IGM low 36

Free light chains, serum
Lambda Free Light Chains at 711.07 High

Kappa/lambda FLC Ratio 0.01 Low

I did not see the results from the 24 hour urine test. I have to go back in two months to get tested all over again blood and 24 hour.

In the meantime, I will see my Rheumatologist as he is checking for autoimmune deficiencies as I have had one infection after another for the past year, (sinus, ear, Kidney and two staph infections).

Does any one know what these (flagged) labs mean?????

Your results mean that there is likely a rogue clone of plasma cells somewhere in your body making an excess of free lambda light chain antibodies. This can be indicative of MGUS or myeloma. Are the lambda light chain results in mg/L or mg/dL? Have you had a bone marrow biopsy?

Hi Dr. Voorhess, it says

Lambda Free Light Chains 711.07 H mg/L Range 5.71-26.30 SJ
Kappa/Lambda FLC Ratio 0.01 L Range 0.26-1.65 SJ


Does my number seem unusually high to you and does that mean I have an increased risk of getting Myeloma with high number and abnormal ratio??

I have not had a biopsy, nor has my hematologist mentioned it.

The level is high but within a range that could be compatible with MGUS or myeloma. With recurrent infections, I would do a complete work-up. Are you anemic? Is your kidney function OK? Are your calcium levels high? Have you had x-rays done to look for bone lesions?

I would do a bone marrow biopsy and appropriate imaging tests (if they have not already been done).

Thank you for your advice. My Rheumatologist is working with my Oncologist and is suggesting a bone marrow biopsy. He also wants to rule out possible Light Chain Disease. No one has mentioned the imaging. I am waiting for my lab work to come back from the Rheumatologist so we can see what else is going on if anything. I had a kidney reflux problem in 1991 and am scheduled to see a nephrologist next week to get a renal scan. I had read that high levels of lambda light chains and abnormal kappa/lambda ratio are indicators that are higher risk for developing Myeloma?
