Hi everyone,
Many of you have heard about the new myeloma treatment ixazomib (Ninlaro, MLN9708), which is likely to be approved by the FDA sometime in the next year or so. The drug is in the proteasome inhibitor class of therapies, which also includes Velcade and Kyprolis.
One of the things that people think may be particularly attractive about ixazomib is that it can be given orally. To be honest, I'm not sure if, when it is approved, it will be given as a liquid solution, or as a capsule. But you won't have to go somewhere to be injected or infused with the drug, like you (usually) have to do for Velcade or Kyprolis.
There seems to be some confusion, however, about the relationship between ixazomib and Velcade. You'll see it sometimes described as an "oral formulation" of Velcade. If people are interpreting this statement as meaning that ixazomib is basically Velcade, but given as a pill or capsule, that's the wrong interpretation.
Ixazomib is a different drug from Velcade. It's related to Velcade, just like Revlimid is related to its "cousins" thalidomide and Pomalyst. Both ixazomib and Velcade, for example, have boron in them, which is somewhat unusual. But they are definitely different drugs.
I've attached images of the chemical structures of ixazomib and Velcade, just so you don't have to take my word on this. The images are from the website "ChemSpider", which is owned and operated by the Royal Society of Chemistry. So I think you can treat the information as reliable.
The reason I'm bringing up this point is just to make it clear that people shouldn't assume that ixazomib is just Velcade in a different form. It's not. And, because it's not, you can expect ixazomib to have somewhat different side effects from Velcade, just like Revlimid and thalidomide have different side effects. You also can expect ixazomib and Velcade to have somewhat different efficacy.

Thank you for this very informative article. I really love articles like this that inform us of new drugs and treatments. I actually keep a "cheat" sheet with all the more common drugs used in treating multiple myeloma. When I read an article like yours, I go to my handy reference sheet. Sure enough, Ixazomib was on my list under the proteasome inhibitors.
My husband received Velcade during his induction chemotherapy and then Kyprolis during his maintenance chemotherapy. He is currently at MD Anderson awaiting his stem cell transplant. I am interested in what type of maintenance chemotherapy they will recommend for him since he still had disease progression after both the induction and maintenance chemotherapy. Your article has given me the impetus to look more closely at ixazomib in case it is an option for him next year.

You can Google the studies on ixazomib, they are called the TOURMALINE studies; there are five of them. I recall reading a couple of articles.
In newly diagnosed myeloma patients, it appears that the study population was about 55, and substituting ixazomib for the Velcade in RVD [Revlimid, Velcade, dexamethasone] got about the same response. However, in relapsed and refractory patients, I think that they reported maybe a little better with the ixazomib; at least in some cases they got a response with ixazomib in Velcade refractory patients.
I think there is not enough data to answer your very good question. What I know about Velcade is that it's very beneficial for t(4;14); with RVD maintenance, it has positive results for del(17p); and it appears that other Beacon readers are reporting that doctors are prescribing Velcade for other cytogenetic abnormalities (CA's). I would not be surprised if it turns out that ixazomib turns out to be similar to Velcade with respect to the CA's. It might not be exactly the same, but probably would be similar (based on the similar overall response).

I am part of a trial that consists of frontline Kyprolis and dex. I had no PN during my 7 cycles of Kyprolis and dex. I then moved to Revlimid, dex and Biaxin (BiRd) for 3 cycles and I am currently on Revlimid maintenance (10 mg) - 9 cycles. My M-spike has vacillated between not detectable and 0.1 g/dL.
As a t(4,14) patient, I expect to eventually be on a proteasome inhibitor-based maintenance program. I am curious if (dare I say hopeful that) another benefit of ixazomib – besides the fact that it can be taken orally – is less incidence of PN?

For t(4,14), it's a little tricky. I have read articles and spoken with doctors who feel that ixazomib is probably as good as Velcade, but that has not yet been proved in studies. I also believe that, for Kyprolis, it also has not yet been proven in studies. Since Kyprolis is better overall than Velcade (for all patients), it is thought that it is no worse for t(4,14). Simply put, if Kyprolis were worse than Velcade for t(4,14), then it would be unlikely that the overall stats would look better.
