Re: Inverted A/G ratio, elevated globulins - next steps?

by Multibilly on Mon Mar 23, 2015 3:23 pm

Well, the things you are going to be interested in are whether the IFE shows the presence of any monoclonal protein, whether the SPEP measures any monoclonal protein (aka M-spike, abnormal protein band, paraprotein, or M-protein) and whether your lambda or kappa free light chain values are out of range. When you have all these results in, these are the primary things that will help determine if multiple myeloma may be a likely suspect.

Okay, I got part of my results back. Now I am confused. I got the Ig quantities back, and it also listed out my Total Protein levels. My total protein is now showing as 7.6 g/dL.

Here are the results (including yesterday's):

Total Protein
Feb 06, 2015 8.1 g/dL
Mar 20, 2015 8.6 g/dL
Mar 23, 2015 7.6 g/dL

Globulins
Feb 06, 2015 4.1 g/dL
Mar 20, 2015 4.5 g/dL
Mar 23, 2015 IgA 182 mg/dL (0.182 g/dL)
  IgM 160 mg/dL (0.160 g/dL)
  IgG 1454 mg/dL (1.454 g/dL)

A/G Ratio
Feb 06, 2015 0.975
Mar 20, 2015 0.911
Mar 23, 2015 ??

Can someone help me figure out if this means that everything is normal, or do I still need to see the SPEP and IFE results?

TXmom,

You really need to get all the results in to see what's going on. I know it is hard to do, but I would suggest not trying to figure all this out as each lab result dribbles in. You'll just drive yourself crazy with incomplete data.

I wouldn't say I am driving myself crazy, I just thought since this was normal. Maybe that means everything is fine now? Is that possible, even with the elevations I had with the proteins and globulins in the past month and a half? That's all I was curious about.

OK, well maybe I meant to say that I would be driving myself crazy (which I've done before when in this same sort of situation ;-) )

It's encouraging your IgG, IgA and IgM all appear to be in normal ranges (note that different labs have different reference ranges, so I'm just assuming these are all normal values on your lab result). But you will still need the IFE, SPEP and kappa & lambda free light chain values to say you are in the clear.

My free light chains were normal.

I'm still awaiting the IFE and SPEP results, but am hopeful. They said I should have those no later than Friday morning, so fingers crossed...

TXmom,

I think your hope is likely justified....wishing you all the best this Friday.
