Increasing lambda free light chain numbers

by angelaw321 on Mon Mar 07, 2016 2:30 pm

My son was diagnosed with multiple myeloma at the age of 17. He has come out of remission and I would like to gain more understanding of what his increasing lambda free light chain numbers mean.

His numbers since June 2014:

Date Kappa Lambda Kappa/Lambda
Jun 17 2014 7.09 mg/L 101.15 mg/L 0.07
Oct 27 2014 9.67 mg/L 133.73 mg/L 0.07
Jan 27 2015 8.45 mg/L 151.42 mg/L 0.06
Apr 20 2015 8.91 mg/L 188.60 mg/L 0.05
Aug 17 2015 10.06 mg/L 228.53 mg/L 0.04
Dec 21 2015 16.44 mg/L 377.59 mg/L 0.04
Feb 23 2016 12.36 mg/L 397.78 mg/L 0.03

Can anyone clarify for me to some degree what these numbers indicate?

He is 23 years old at this time and I would like to understand the increasing lambda free numbers and what the ratio means so that I can adequately monitor his labs. The doctors tell me it's nothing to worry much about at this time - but he's my son.

Hi Angelaw321,

Welcome to the forum. Your son was diagnosed at 17? I can't even begin to imagine having a child with multiple myeloma at such a young age.

When you say "he is coming out of remission", what was his earlier treatment that got him into remission and how long ago was that treatment? Also, is he currently on any treatment of any sort (maintenance or otherwise)?

Also, what has his M-spike done over this same period of time?

Answering these questions will help folks on this forum better help you.

I track my multiple myeloma though the serum free light chain (sFLC) assay as well. The numbers you posted show a clear upward trend, as both the lambda light chain and the ratio are increasingly out of the normal range. This indicates that the myeloma is active. I assume that you were aware of that since you said he "has come out of remission".

The increase has been slow but persistent. What this means is that he will likely have to re-start treatment, if he is not already re-starting it, or his existing treatment will have to be modified to stop and reverse the progression.

Hi Angelaw321,

I remember reading about your son after I was diagnosed. I am very sorry to hear that he is coming out of remission. I had seen last year that he was still in remission. I sincerely hoped he would remain there.

I was diagnosed in my early 40's and that is considered very young for myeloma. I pray things will go well moving forward.

Mark
