Increasing doses vs changing meds at relapse

My husband is in relapse (2.5 years post transplant, 17p deletion). He feels fine, no pain, all labs are good with the exception of his m-spike now being at 2.0 . He had a full skeletal series last fall that was fine (m-spike at 0.7 at that time).

He has been on Revlimid 10mg and Velcade every other week for the last 1+year. His doctor has now given him the option of increasing his Revlimid to 25mg and upping the Velcade to every week, and also adding in dex, OR changing to Pomalyst [pomalidomide, Imnovid] & Krypolis [carfilzomib]. He has chosen to do the VRD.

Is upping the dose likely to bring the m-spike down, or is it more likely that he is no longer responding to Velcade and Revlimid?

I think it will take time for your question to be answered. multiple myeloma has no norm, I guess that's the way to put it. If he doesn't see some progress in the VRD he will still have the Pomalyst [pomalidomide, Imnovid] and Kyprolis [carfilzomib] to try.
