Increased lamba free light chain

I have been on Kyprolis, Revlimid, and dexamethasone for about 10 months and have been in complete response about 6 months. For the last 6 months I have had the treatment every other week as a kind of maintenance. My last treatment was Sept 21st. Since then, I was out of town for 2 weeks and in the hospital for 2 days with an infection, so I have missed 2 treatments. After getting out of the hospital I received the test results from 24-hour urine. I was concerned that my lambda free light chain went up to 82.93 and my ratio is 0.10.

I messaged the doctor my concern and she relayed a message through her nurse that "I should not worry about these results". It is difficult for me not to worry about the results until I know why and the doctor is out of town and cannot be reached until next week.

Could it be the lambda free light chain went up due to lack of treatment, but I can expect it to go back down upon resuming treatment? Can the infection cause it to go up?

Measuring and tracking free light chain values via a urine test is quite unreliable. Did you have a serum free light chain test and did it show an increase in your numbers?

Not sure. But my paraprotein is 0.3 and has previously been zero.

Suggest you go back and look and see if you had a serum free light chain test. A 24 hour urine test is good for several things, but accurately measuring the absolute values of free light chains ain't one of them. Did the 24-hour urine test detect a urine M-spike (Bence Jones proteins)?
