Of those people who started outpatient but needed to be admitted, how did that transition work? Go to the ER? Specially orchestrated through the outpatient clinic? Also, how did release work?

Hi Lizbet, If you have a problem as an outpatient when you are a patient at a bone marrow transplant clinic, definitely don't go to the ER. yOU would need to contact the clinic and they would call you in for an appointment and re-admit you to the clinic if necessary. They would give you a list of problems for which they would like you to call, including fever over a set temp. Hope that helps!

When you have your SCT as an out-patient, you are seen on the transplant unit every day for some time. When my husband was about 4 or 5 days post-transplant he was quite nauseas and not taking in enough fluids. When he went for his daily transplant visit, his blood pressure was a bit low from being dehydrated. They had him stay for an hour and ran a liter of fluids in through his iv line. They wanted to admit him, but he wanted to go back to where we were staying. When we got up to leave, he fainted, and they admitted him. The out-patient transplant visits were on the same unit as the patients that were being done as in patients, so admitting him was just a matter of changing him to a room that had a bed in it. This was at the Mayo Clinic in Rochester.

That's right...you would be seen on a daily basis Lizbet, although maybe not on weekends. I was treated as an outpatient since I was well enough for that. I was only in the transplant unit for two days before being sent home. Many patients do stay in hospital though...it just depended on the situation for each patient. However, I was readmitted due to having a fever about ten days after my stem cell transplant. I also received hydration through my IV line at times during my daily app'ts. These app'ts were actually quite time consuming, since my blood needed to be tested and also the medical staff always gave a very thourough check up. This was at the Tom Baker ctr. in Calgary.

When I had my transplant in Omaha at the Universityof Nebraska Medical Center I had the in Patient setting it was a lot safier because I ended up getting a blood infection and a urinary infection so the treatment took place right there. After I had recovered from those and my White blood count increased I was released to a motel that is in the clinic area so all I had to do is walk down to the clinic to do my tests. I was able to leave this area as I wished but took care as to where I visited during the day. I had a caretaker with me also and they are able to stay in the room with you. The meals in the hospital are ordered to what you want and when and when I was in the outpatient mode was able to bring in groceries,eat at the cafeteria, or eat at the resturant of my chosing. I am doing fine almost 10 months from transplant and starting Revlimid and Zometa. I had a choice to stay near by in a local motel and chose not to because the in house motel was cheaper and aloud easier access.

My husband had his auto done as an inpatient at Simon Cancer Center. At harvest, they told us that there was a possibility that it may be done outpatient. After lengthy discussion we decided that we were not comfortable with that. Luckily, everything worked out. I think we are both glad that he spent the two weeks in the hospital. The nurses and doctors were trained to give him the care that he needed at the time.

I did out patient in Denver two months ago and had no issues. But, you need full time caregiver to do this. Clinic was great and we rarely stayed more than two hours each day. Going home each day was great, lost minimal weight, no depression, a great experience in my opinion. Jerry

My father did his auto outpatient. I was super anxious about this going in, but the fear I had from the scenarios I created in my head was completely unfounded. He would have gone crazy in the hospital-thankfully he never was sick enough-hospitals are not a healing environment-especially for the immunocomprised. His dog got to be with him at my place, it was quiet, had cable, comfy bed.
Did require daily trips to the clinic for a couple weeks.
I highly recommend outpatient. The patients are followed so closely, if the slightest thing is off they can be admitted. Best wishes-it's not as bad as you are anticipating.
