Is it normal after the chemo the uninvovled mmunoglobulins to drop under the normal range? Thanks

I don't know if it's the case for all forms of chemo, but in my treatment with CRD (Carfilzomib, Revlimid, and dex) my IgM and IgG were both below the normal range. IgG hovered between 500 and 600 (normal range is 620-1520), and IgM between 10 and 20 (normal range 50-370).

Thank you for your answer Kevin. I received VRD-PACE. I have IgG multiple myeloma. What type do you have? Where are you treated? I have to travel from Greece to UK and Germany in order to be treated

I have IgA multiple myeloma, and am just finishing a two year treatment with CRD in a trial study for newly diagnosed patients at the University of Michigan in the US. On a related note, my IgA when I started treatment was at 3400 (normal range 40-350) and has hovered between 45 and 60 ever since I reached CR. Sorry you have to travel so far for your treatment.

Hi Kevin, I just saw your post and just wanted to say it is AWESOME you are close to maintenance and you are maintaining your CR! What a milestone and I know what a pain the whole thing has been from my own experience. Way to go!!!!! Terry L. from the other CRD trial on the east coast!

Dear Maria,
It is fairly common for the univolved immunoglobulins to be below the normal range under your circumstances. They often are at initial diagnosis, even before chemotherapy is started, due to the fact that the myeloma suppresses the production of normal immunoglobulins by non-cancerous plasma cells. In addition, chemotherapy can have a negative effect on normal plasma cells in addition to myeloma plasma cells, leading to a temporary decline in normal immunoglobulin production. VRD-PACE is a highly immunosuppressive regimen, so I am not surprised to hear that your immunoglobulins are now low. Once you have achieved a good remission and therapy becomes less intensive (for example, observation vs Revlimid maintenance therapy), I would expect that the uninvolved immunoglobulins will creep their way back into the normal range.
Congratulations to Terry and Kevin! The CRD regimen is looking really good -- your experiences are a testimony to the effectiveness of this combination. Keep up the great work!
