Im trying to sort it all out?

by Restlessrebel on Sun Aug 19, 2012 1:06 pm

Hi everyone, I am very happy to have found this website and you. I recently went for routine labs as I do every year and they had detected a high level of protein in my urine. My doctor sent me in again to do immunofixation test on my urine and that came back with "Monoclonal Free Lambda light chain. I now am being tested at a hematologist/oncologist office. They have drawn blood and I did the 24 hour urine test. I go next week for results.

I am not sure what to think. My only symptoms have been recurring infections (mainly sinus, ears) My liver enzymes were slighted elevated and my ANA titer came back at 1:160.

Do you think this sounds like MGUS?

Thanks for your input :|

Hi Restlessrebel,

I don't have enough expertise to really answer all your questions, but I can give you some pointers which might be helpful.

First, you may find these previous discussions here in the forum useful:

https://myelomabeacon.org/forum/what-does-a-value-of-1-mean-for-m-protein-t714.html
https://myelomabeacon.org/forum/spep-monoclonal-pattern-t1157.html

The key thing that will really determine whether you have MGUS is, I believe, the level of your monoclonal protein (mspike). Have the initial test results given any indication of that level yet?

Second, as you've probably already found out, the ANA titer value that you got is elevated, indicating that you may have some sort of autoimmune system disorder. I believe it's often the case that people with autoimmune disorders also develop MGUS.

Are you also having additional tests done to understand what the source of the elevated ANA titer might be?

I hope the test results you get next week clear things up. Most of all, I hope all of this has been some kind of weird false alarm! :-)

Please let us know what you find out.

Ricardo

Dear Restlessrebel,

MGUS is exceptionally common, more so than multiple myeloma. Whether you fit the bill of someone with MGUS is difficult to say with the information you have presented. If the amount of monoclonal free lambda light chains is low on 24-hour urine testing, there are no kidney problems, anemia, or elevated calcium levels, and no bone lesions on x-rays, odds are that you have MGUS.

Let us know how things go with the remainder of your work-up.

Good luck!

Hi Ricardo and Pete, I will find out tomorrow. Thank you for your replies. I went back and looked at my labs. The abnormal protein band 1 is listed as "2". I do have an underlying kidney problem already from a reflux problem I had. Surgery was done in 1991. I am also seeing a Rheumatologist for the high ANA result. Again, thank you for your information.
