Is hypercalcemia an early sign of multiple myeloma?

Hello everyone.

I am 26 and I know I am too young for multiple myeloma. But my blood test showed disorder in calcium, so I'm worried!

24 November, 2016 Went to the ER after workouts :

wbc 11400
rbc 5.7
hemoglobin 45
hematocrit 15.6
creatinine 1
calcium 10.2

30 November, 2016 (after 6 days had a [fasting] blood test)

Creatinine 0.8
Calcium 10.8 (increased 0.6 in 6 days)

11 December, 2016

Creatinine 0.8
Calcium 10.1 (came back to normal range in 10 days)

25 December, 2016

Wbc 7000
Rbc 6.5
Hematocrit 48
Hemglobin 16.3
Creatinine 0.8
Calcuim 10.1

Please note that vitamin D, TSH, are in normal range.

Is hypercalcemia an early sign of multiple myeloma? If you have hypercalcemia caused by multiple myeloma, can your calcium levels return to the normal range after 10 days without any treatment?

Hi Bringo001,

There are a lot of things that can cause increased calcium including dehydration (you mention that you had tests after your workout) some medications, eating too much calcium, or taking too much Vitamin D. My calcium level (I don't have multiple myeloma) also tends to be around 10.1 but my doctor is not worried about it.

Hey Lynn,

Thank you for replying. I really appreciate it,

But my calcium rose 0.6 mg/dl in 4 days. It was a fasting blood test. When I did it after workouts it was 10.2 and this number didn't really worry me.

But what I am concerned about is the 10.8 and then it came back to 10.1 after 10 days!

Hi Bringo,

Fasting isn't going to really impact your serum calcium level that much. But your consumption of dairy products and other high-calcium foods a few days or a couple of weeks before your test, along with things like dehydration or over-hydration, could easily impact your calcium level (per my oncologist).

Also, these aren't huge swings in your calcium level. My calcium level routinely bounces between 9.4 and 10.6 mg/dL, and it causes no concern with my doctor.

More importantly, does your doctor think you have an issue to be concerned about?

Second, no, actually my doctor said that it's alright since it came back to normal range. But I am afraid, which is why I asked here.

Hello,

Please excuse my English because I am French educated .

I am a 26-year-old male. Diagnosed in October with a fatty liver after having a biopsy that showed more than 50 steatosis .

And here the story begins.

I started to have several tests every week to see how my ALT and my AST are doing with my new life style (a healthy diet and workouts).

On 24 November, 2016 had a blood test after workouts that showed normal calcium 10.2.

On 30 November, 2016 had a blood test that showed hypercalcemia. My calcium level bounced from 10.2 to 10.8 in 6 days!

On 12 December, 2016 had a blood test that showed that calcium returned to normal level (10.1)

On 21 December, 2016 had an appointment with an oncologist who said to me: "Go enjoy your life. You're still young. And don't come back to me ever. Don't think about it. It's nothing."

And he ordered a blood test and calcium remains normal 10.1.

A few days later, I had another appointment with another oncologist and he said you should see an endocrinologist.. Your problem is somewhere there. Not here. And I repeated my calcium test. And it was normal again 10.0.

On 28 December, 2016 and when i was laying on my bed and putting my hand on my head, I heard a weird sound in my bones (I guess it was in my left arm or shoulder) .

So next day I had the calcium test again and it was normal 10.0.

Then I had an x-rays for my shoulder and arm. And it showed nothing abnormal.

On 3 January, 2017 (after 4 days since my last test ), I repeated the tests that the first oncologist ordered to me, and my calcium was 10.6! (again + 0.6 in 4 days).

Next day i went to an endocrinologist. He ordered some tests. One of them was free calcium test. He said my results are normal with serum 1.37 and ionized calcium 5.47, lab range is 1.17-1.40 and 4.64-5.60, but I saw on the Internet that this level is considered slightly high. And the next day told me that there nothing to worry about.. I asked him if I should go back to the oncologist, he said no, all is normal. Go live your life and forget about the calcium!

I called my oncologist, he asked me if the endocrinologist said that I should come back to him. I said no. He said ok, forget it then, and don't do your calcium tests again!

And here I am.. Afraid of having misdiagnosed multiple myeloma and no one is giving me a clear answer of the reason why my calcium levels are bouncing like that! And I really don't know if should forget about it, like my doctors said, or be concerned of this problem ..

Had an x-ray for my feet (because they hurt one month ago) and my cervical spine. And had a CT scan for abdomen.

I am not consuming dairy products and other high-calcium foods. It's been 1 month and I am starting to feel some mild pain in my chest and back sometimes.

Bonjour Brigo, ca va? i hope that at your young age you do not have multiple myeloma, but if you were to be diagnosed, it would be after having some other medical tests too.

You would need to have your SPEP tests done (immunoglobulins, monoclonal proteins), a skeletal Xray survey or other scans to check for breaks or lesions on your bones; creatinine tests for your kidney function; and white blood cell counts to check for anemia. There is an acronym for these four tests (including calcium), CRAB. Hope that helps!

Ca va Nancy?

I had my calcium test yesterday and it was 9.7! Does it bounce up and down in multiple myeloma, or would it always be elevated?

Hey,

What should I do next? I am really confused.

Last week i felt that my body was dehydrated although I was drinking a lot of water. Went to the emergency room and creatinine was 1.2 (it was 0.7 two days before) and calcium was 10.4 (was 10.2 two days before).

Oncologist refused to order myeloma tests but I insisted. Protein in 24 hours urine test (6),
SPEP normal, immunofixation normal, ESR 2, hemoglobin 16.7, hematocrit 50, Bence Jones negative, calcium remains 10.2, and creatinine 0.8 the second day

I insisted on a bone marrow biopsy, but the doctor refused and i did a bone marrow aspirate. That showed 0.1% plasma cells only in flow cytometry. So the doctor said it's enough to rule out myeloma! And they think that I am overthinking and I don't have it.

I feel pain in my back sometimes and doctors are refusing to do a bone marrow biopsy because they believe I don't have myeloma. I am only 26. Are the doctors right?

Bringo001,

I'm not a doctor, but I would say that your doctors are correct in saying that this is not multiple myeloma. Having a normal immunofixation test result, no urine Bence Jones proteins, coupled with only 0.1% bone marrow plasma cells makes it pretty clear that this is not multiple myeloma. Also, your creatinine and calcium numbers aren't worrisome (these numbers will all normally fluctuate from day to day and can be impacted by many things) and you have a very good hemoglobin level. I would be looking for another diagnosis to explain your back pain and be thankful that you aren't dealing with multiple myeloma.
