I would be glad to have some advice on how to handle the side effects from Kyprolis (carfilzomib).
Around 5 hours after injection, the patient suffers from fever, chills, joint pain, muscle pain, etc for the next 8 hours. He already drinks a lot of water, rests in bed, and takes pain killers. It is so painful to see him going through this pain weekly.
Does it help to reduce the side effects if we request to slow down the IV speed?
The IV finishes in 10 minutes now.
Thank you very much.

Re: How to reduce Kyprolis side effects post infusion?
For what it’s worth, I’m completing my second cycle of twice-a-week Kyprolis infusions as part of consolation after an autologous stem cell transplant (ASCT) before moving to maintenance. I’ve had a really good response with minimal side effects. Even though I work hard to stay hydrated, I still get a bag of fluids prior to the Kyprolis. Asking for fluids and slowing the infusion rate might help and would definitely be worth asking about.

10 minutes is pretty fast. My infusion is 30 minutes, and it's my understanding that's normal (although I certainly could be wrong about that).

Re: How to reduce Kyprolis side effects post infusion?
I have been receiving Kyprolis for a few years now, as maintenance. I also take a low dose of dexamethasone on both infusion days and receive Zofran for nausea. The dexamethasone is said to help with the side effects. At first, I suffered with many different side effects, and then they lowered the dose and now I do quite well other than some fatigue a couple days after the treatment.
My infusion are about 15 minutes, and I receive extra hydration.
I wish you the best.

Re: How to reduce Kyprolis side effects post infusion?
My husband had a port put in and was given Kyprolis for consolidation therapy. He always got a steroid infusion prior to the Kyprolis, and it was run about 20-30 minutes. First infusion side effects were scant. After that none.
Hope this helps.

Re: How to reduce Kyprolis side effects post infusion?
For what it's worth, I’m on my third cycle of Kyprolis. It's definitely having an impact on my light chain levels, but knocks me out for days afterward: exhaustion, breathlessness after mild exertion, etc. I do 20 mg of dexamethasone once a week. Can't take much caffeine at all, so I'm not sure what might help the “flattening”. Anyone had success with anything?
P.S. - I had my infusion slowed down because it caused a burning sensation at the infusion site otherwise.
Had hydration via saline for the first couple of infusions, but none since. Doesn't seem to matter to side effects either way.

Re: How to reduce Kyprolis side effects post infusion?
Hi,
I receive Kyprolis each Friday and Saturday, 3 weeks on and one week off. One of the things that has helped is taking Claritin (loratadine). I'm not sure why it works, but it does for me. Also, I have them dilute the medicine over 30 minutes. The only side effect I get is shortness of breath, which at times is tough. I have an inhaler at home.
Best of luck to you.

Re: How to reduce Kyprolis side effects post infusion?
I just received my first treatment of Kyprolis. I was given 20 mg with an hour or 250 mg of hydration before and another 250 mg hydration which took an hour after the infusion. I think it took about 30 minutes for the Kyprolis infusion. I took 20 mg of pills of dexamethasone before the infusion on both days for a total of 40 mg.
The only side effect I have started two nights later with really bad constipation and mild upset stomach. I started taking 2 senna pills (laxative) and a Prilosec (omeprazole) pill daily. I did feel a little tired around day 2, but that passed.
I am 47 and in very good health with no other health issues besides cancer. I also stretch ever day and do strengthening exercises almost every day. My work outs are limited right now because of the recent back surgery-read my bio. I also drink nothing but water and drink about 100 oz of water a day.
I am also taking cannabidiol oil, curcumin, and several other vitamins and supplements. I don't eat any sugar. I eat organic and non-GMO. I am also a vegetarian.
My M-protein started at 10 and got down to 4 but stopped there with Revlimid, Velcade, and dexamethasone. I am now starting Kyprolis and dexamethasone and hope that it gets my M-protein to 0 and the cancer in remission.
