Has anyone had any experience with a high potassium at the start of treatment with Velcade, cyclophosphamide, and dexamethasone (CyborD)? They called us at home after his treatment today and blood draw and said my husband's potassium is high (5.7). They have called him in a one-dose med and we are to go back tomorrow to have it checked again. If it is not better, they may admit him.
I have done some research and tumor lysis syndrome comes up. It states that the chemo is destroying the tumor cells so quick that it is overloading the blood with potassium and with his kidneys compromised from the myeloma it is building up.

Was the staff able to help with the possible cause of the high potassium? If he has elevated creatinine, it may also be a contributing factor. Is he eating a low sodium, low potassium and low phosphorus diet? Is he able to consume sufficient fluids each day (ie, 2+ liters depending on body mass)? The DaVita website has a good number of suitable recipes. Hope this helps.

I asked him about his fluid intake and he did admit he had slacked off this last week, so I told him this may be his issue. The pharmacist told him when he picked up his script last night to make sure he is drinking plenty of water as well. She told him to push water last night and this morning. His appointment is just after lunch today so maybe between the meds and that it will be down. They did not mention any other labs out but, at the start of diagnosis before treatment, his kidneys were severely compromised with a creatinine of 5.0. It was down to 2.6 two weeks ago. I am going to ask for printouts of yesterdays labs today when we go.

His potassium came down to 3.7 with meds and pushing fluids. They told him to remember to keep pushing those fluids even though he feels better.

Glad to hear the medication worked. We were advised to also make the dietary changes (a/k/a the kidney diet) to further support a lower creatinine level. Do you have a nephrologist involved in his case?

No, but his creatinine is down to 1.6 from 5.0 at it's highest, but I will check into this diet. Thanks.
