High kappa / lambda ratio - have I relapsed?

My recent blood test reveal

Kappa / Lambda Ratio = 6.52
Kappa Light Chain = 53.79
Lambda Light Chain Free = 8.25

Have I relapsed?

I was diagnosed with primary amyloidosis in May of 2013. Treatment of Velcade and CyborD. Side effect was peripheral neuropathy both feet.

Anyone here ever been diagnosed with amyloidosis?

Dear Bullitbob,

It will require a bit more information to determine if you are relapsing:

  1. Is your amyloidosis kappa subtype?
  2. What were the levels of your kappa light chains and kappa/lambda ratio at diagnosis, and what were they after the CyBorD treatment?
  3. What are the results of your SPEP, serum immunofixation, UPEP and urine immunofixation, if done?
If your free kappa light chains and kappa/lambda ratio normalized after treatment and are now rising, then this is concerning for relapse. But the labs should be repeated at least once to confirm and taken into the context of the other labs mentioned above, as well as how you are feeling and how your organs (e.g., heart, kidney, liver) are functioning.

You should discuss these findings with your oncologist, who can determine if you are truly relapsing or not and recommend appropriate therapy if you are.

Thank you Dr. Cohen.

I have no M-spike. Everything else is normal. This is the second set of labs. I had these done almost 3 weeks ago. Have not heard from my oncologist. I am not sure what "kappa subtype" is.
