Niki,
I'm at a loss to comment. Maybe some others on the forum have been through this sort of thing themselves.

Re: High kappa-lambda ratio due to low lambda level
Hi Niki and Multibilly,
Is it possible that the test results Niki got are showing that she has
1. Signs of a light-chain only clone, i.e., a clone that throws off only monoclonal kappa light chains, rather than throwing off complete immunoglobulins and excess free light chains, and
2. The kappa light-chain only clone is present to just a limited extent, so it is not causing an above-normal kappa free light chain level?
Recall that the serum free light chain test measures both healthy and monoclonal free light chains.

Jim,
I was sort of thinking along your same lines, but I wasn't entirely sure of myself.
Niki,
Did your serum protein electrophoresis test (not the serum immunofixation test) measure any sort of a serum M-spike?

Re: High kappa-lambda ratio due to low lambda level
The serum protein electrophoresis test is negative for M-spike, fortunately. Otherwise the oncologist would be sure that this is a relapse. He is still thinking about it due to the negative bone marrow biopsy and a negative minimal residual disease test that I had in July.
He believes that the findings are associated with some infections that I had lately, but who knows, time will tell.

Re: High kappa-lambda ratio due to low lambda level
Indeed, time will tell ... such is the nature of this beast.
