Hi There, I am trying to figure out how my Lambda and Kappa Free Light Chains levels other people have to compare to mine. I just cannot find references on what researches consider to be very high in in people who have MGUS.and are out of the normal range in mg/L.

The normal ranges for Serum Free Light Chains (Freelite test) are: Lambda FLC, 5.7-26.3 mg/L, Kappa FLC, 3.3-19.4 mg/L while the normal K/L ratio is .26-1.70.
Those with high Kappa numbers will usually have a high ratio while those with high Lambda readings will have a ratio below normal. Over a period of time with repeated testing the stability of the K/L ratio is usually as important if not more so than the K or L number itself.

My first test for free light chains and my Kappa is 134.94 and Lambda is 7.95... what treatment would be recommended... anyone else have such a high Kappa?... does this mean protein in the urine?

Hi Bob,
Based on the FLC assay results it sounds like you have IG kappa myeloma.
Initial recommendation for myeloma varies depending on the myeloma expert.
A common initial therapeutic regimen, is a low-dose steroid, 8 cycles of a protease inhibitor along with an immunomodulator.

Thank you Suzierose.... I am still kind of in the dark, what does 8 cycles of protease inhibitor mean.. will the 8 cycles be administered via IV ? and does this mean administration of a series over a period of weeks... how many administrations of the drug would constitute a cycle?
I live in Oklahoma and am considering going to Arkansas...but if the treatment (Gold Standard) is the same everywhere then I might be better off just staying in Okla City... although I can certainly tell where I am going is not a Myeloma specialty center. I know that treatment is dependent on what is discovered in my bone biopsy and aspiration.... THE place in Arkansas doesnt accept any findings from other clinics..so I dont want the biopsy done multiple times... will save that for hopefully a few years down the line... ( after the stem cell treatment) So sometimes people can have this treatment that you identified and not have to be on daily oral maintenance... I know each situation is different. I have a first hand acount of a gentleman that had stem cell in Dallas in about 2006 and only recently did he have to start the oral meds (Revlimid).

Don't get ahead of yourself. Do you have ANY symptoms? Anemia, bone lesions, etc. Have you had a skeletal survey or PET scan?
The bone marrow biopsy should be done to confirm, along with IGG and M-Spike, where you're at.
Personally, I would think stem cell transplant is at the far side of treatment. I for one hope to remain smoldering for as long as possible. And, hope that a new treatment comes along (and they're working on them) that will have less side effects than what's out there now.
If you're smoldering, I'd look into taking Curcumin. There have been trials showing positive results. My Kappa was at 84 the first time I looked at test results . It dropped to 64 a few months later. Don't know if was the curcumin but I'll continue taking 8 grams/day.
