Hello,
Dad is undergoing ASCT. As of today, it is D+13, and his counts are as follows: hb 9.0, wbc 7.19, platelets 30. Doctors are concerned with his febrile state, which is basically a constant fever, of 38.2 deg centigrade. The body temp has been hovering at around 37.5-38.5 deg centigrade since the day he autografted, which is about 3-4 days ago. Doctors have been taking his blood samples everyday, but all infections came out negative (also as of this morning's test).
Please help answering my questions, (1) what may cause his fever? Doc may order PET scan if his temp if his temp does not go down in 2 days, and (2) is this associated with a very fast autograft? Dad had zero infections and mouthsore, and only vomit once during this course. He can go home in 3 days if the temp goes down.
Thanks'

Re: Fever after stem cell transplant
Sorry to hear about your father's situation, tpt.
In reading about what is happening with your father, we recalled a news article we published a few months ago that you may want to review. It's about a complication of transplantation called "engraftment syndrome". Here's a link to the article,
https://myelomabeacon.org/news/2013/07/12/risk-factors-engraftment-syndrome-multiple-myeloma/
There's also a discussion on the same topic here in the forum which you may find helpful:

Finally, you may also want to check out this older article about the diagnosis and treatment of engraftment syndrome,
http://www.nature.com/bmt/journal/v27/n9/full/1703015a.html
It argues that treatment with steroids may be advisable. This could be an option to discuss with your father's physicians.
Good luck, and keep us updated on your father's situation and what steps were taken to deal with his fever. It will be very helpful for all the other Beacon readers.

Re: Fever after stem cell transplant
Thank you.
I am going to see his Doctor on Monday, and I will ask him in detail what's going on. From those article links about engraftment syndrome (hey, thanks a lot!), Dad only has the fever symptom, i.e. he does not have any rash as well as the other symptoms. The article said that a person must experience at least two major symptoms and one minor symptom to be considered as having the engraftment syndrome... so I don't think it is what Dad's having. Also, as of today, his body temp never exceeds 37.5. I hope it ends right here, because if he does not have any fever for the next two days, he gets to go home!
Anyway, I will post the updates... let's hope he does not have to do the PET Scan! Thanks for your help!

Re: Fever after stem cell transplant
Hello,
My Dad was discharged last Tuesday, after his temp remained normal for 2 straight days. He did not take anything except acetaminophen.
Doc said the fever was self limiting in nature. He did get a chest x ray, and Doc found a weird object on his lungs, and told us to do a CT scan. Doc said that it has nothing to do with the fever, though. It really worries me until we do a CT scan. But he's no longer in the hospital, and that's what's important--for now.
