I have been seeing improvements in my labs and this last lab included the regular CBC w/auto diff, CMP, SPEP, IGA, IFE, total protein and this time I also had free lambda serum, kap/lam ratio, free kappa serum.
There are also comments posted for the SPEPand IFE tests. My SPEP comments say that there is no longer a protein spike evident. But the IFE comment says faint spike still present. Also in the tests comments it says undetected antigen excess is a rare event but cannot be completely excluded. This I really don't understand.
Just wondering if someone could interpret these comments along with the numbers that I will show below. I am a little concerned about the free lambda being low. When I went to get my Velcade on Thursday, the nurse gave me a report with a happy face on it. I didn't think much about it till I got home. It shows wbc, hgb, plt normal and something I haven't seen before was anc, which shows normal as well. This will also be my first labs showing my RBC normal.
I get my labs online but don't get the full details that my oncologist receives. Just got these labs this morning. Also, sorry for first posting in treatments and side effects. Chemo brain, I guess.

Sounds like you are doing well which is great news!
The SPEP is less sensitive than the IFE. So because the protein level is declining, it is no longer detectable by SPEP and only faintly detected by IFE. Hopefully, moving forward it won't be detected at all! As for the comment that "antigen excess" can't be excluded, this is a disclaimer that a negative result can be falsely negative in the setting of too high of a protein- but that does not appear to be the case since your protein levels are low accross the board.
The ANC is your neutrophil count, a specific type of white cell. Nothing to worry about there either since that is normal too!

Thank you so much for helping with these labs. I was taking Revlimid/dex until I got a severe rash so my oncologist started me on Velcade/dex and since then my labs have gotten better.
I'm going tomorrow to USC Norris to go over these results and also discuss a future stem cell harvest and transplant.
