Faint IgG lambda band after radiation for plasmacytoma

Hello all and thank you so much for being the most reputable source of sound information on the net. Additionally let me thank all of the loved ones and patients on this website making a difference.

I am 44 year old white male in good health minus a solitary plasmacytoma on my L5. I had only an elevated free light chain of about 120. After 5 weeks of radiation @ 50 grays, I was done and my free light chain level was down to about 47. 5 months later it jumps up to 68. All scans show no other lesions, but there are a few small spots they are "watching".

Biopsy and bone marrow biopsy determined plasmacytoma in May. I am on Zometa and nothing else.

Only areas of concern for me on my labs are as follows:

Serum Immunofixation

Faint band in IgG and lambda is present against a dense polyclonal background. While this may represent a reactive / inflammatory process, a developing plasma cell disorder cannot be excluded. Clinical correlation is suggested and if indicated a repeat of the immunofixation testing in 3-6 months.

Kappa serum free light chain 11.3 3.3-19.4 MG/L
Lambda serum free light chain 67.9 5.7-26.3 MG/L
Kappa/lambda serum FLC ratio 0.17 0.26-1.65


My obvious concern is that:

A, The radiation was not effective,
B. I have MGUS or multiple myeloma, or
C. This may just be somewhat normal.

My doctor is not as concerned as I. I am scheduled to meet with a multiple myeloma specialist at the John Theurer Cancer Center.

Given these are the only abnormal results, anyone have anything similar or any ideas what is going on with me?

I am a bit on the young side to be having multiple myeloma, so I am really unsure. I am retired military and was exposed to some nasty chemicals and now this ... I have no idea.

Hi Asublime,

Welcome to the forum, but sorry to hear that you are dealing with this condition.

What did your serum immuonfixation test state before you started treatment? Having mono­clonal protein in one's blood is fairly common in solitary bone plasmacytoma patients and is the reason I am asking about what your immunofixation results were pre-treatment.

What was your bone marrow plasma cell percentage back in May?

How many total radiation sessions did you have over the 5 weeks? I believe 20 or so sessions is the typically recommended treatment for people with solitary plasmacytomas of the bone.

In any case, I'm very glad you are going to see a multiple myeloma specialists for follow-up.

Hi,

I did 25 treatments. Was fun! I am not sure of your other questions since I have only online access to my blood work. Additionally, it only goes back to post treatment.

Just concerned because my free light chains are creeping back up and my FLC and my kappa ratio is getting lower and lower.
