I have been on Velcade, Revlimid, and dex since February 2. On my second kappa light report after three weeks, my kappa light number had decreased from over 1,000 to now just 199. The doctor thought that the decrease was very positive. Is this the normal decrease when starting the induction therapy? Also, how hard is it to go from 200 to below 20, which is what I have read is the high side of normal.

Re: What sort of kappa light chain reductions to expect?
It varies by patient, but your response has been great. Typical treatment is at least 4 rounds. Like you, my numbers dropped quickly, but after 4 rounds my kappas got down in the teens. After a stem cell transplant, they settled into single digits. Just recently started elevating after three years. I'm now taking Pomalyst.

Re: What sort of kappa light chain reductions to expect?
Hello Aclinkboca,
Seems you had a good response to the treatment. My kappa light chain was 5500 when diagnosed in June 2015. Also on Velcade, Revlimid, and dexamethasone (VRD) starting in July. Am down to 133 from last blood work, which is up from 122 the previous month. I am into the ninth month of treatment. I am also looking for the under 20 number, but I believe it gets harder to reach the closer we get.
I was 71 years old at time of diagnoses.
Best of luck on your treatments. Jim

Re: What sort of kappa light chain reductions to expect?
Thank you for your replies. I am starting my fourth round of dex, Velcade, and Revlimid today. My kappa light numbers are all the way down to 61 now. I am preparing for a stem cell transplant in a month.
