Elevated creatinine - did not expect that today!

At today's appointment I got a bit of a surprise. My creatinine is elevated. Never been over 100 before! Today it was. My specialist had a new fellow working with her who mentioned she is a kidney specialist. Lots of questions about how much water I drink, medications I take, etc.

I think I am doing all the right things. I have been making efforts like avoiding kidney-toxic Advil (ibuprofen) (most of time), and I drink lots of water. I had a kidney issue during my transplant from the melphalan. Maybe it caused more damage then we thought, or maybe it's myeloma progression.

New rounds of testing and follow up in a month. Follow up bloods, 24-hour urine, and an MRI in the next few weeks. I have never had an abnormal free light chain test, but I have IgG lambda-type myeloma.

Hello Alyssa,

What are the units being used for the creatinine level? I thought the typical units have the results being in the 0.4 to 1.1 range for people without kidney issues. Those units would be mg/dl.

I hope the creatinine issue is just a blip due to some weird combination of circumstances. Were there other markers of kidney health, such as your BUN level, that your doctors checked and discussed with you?

Please keep up us updated on what you find out. I'll be crossing my fingers that this means nothing in the long term.

Hi Ian,

I'm pretty sure that most labs in Canada use the international system of units (SI) standard of umol/L for measuring serum creatinine. 100 umol/L = 1.13 mg/dL.

Normal serum creatine levels for females in SI units are typically 50-90 umol/L, but they will vary from lab to lab.

Oh, sorry I did not post the units. I am in Canada, Multibilly is correct.

Due to my young(ish) age of 38, my doctor's fellow said I should be lower then the range cut off, about 70 would be a healthy number.

I do have vague symptoms like more frequent headaches, more fatigue and aches and pains with my slow progression, but no red flags have shown up in tests or labs. Only real sign has been a slowly increasing M-spike.

Myeloma can affect our kidneys or the meds and treatments can impact our kidneys. Myeloma is just so sneaky.

Hi Alyssa and all,

On my lab flow sheet, the normal range for creatinine is 35-100. I live in Alberta, and the normal range for creatinine clearance (est.) is 12000 - 23000. I don't know what the units are, but the creatinine range sounds like yours in Ontario.

Thanks, Multibilly and Lys, for clarifying the units question. I should have known better. I just assumed that, when the units didn't seem like the usual mg/dl, that perhaps Lys was reporting something else, like eGFR.
