I have been treated for kappa light chain multiple myeloma since March with Revlimid and prednisone. Every month until now showed a decline in my light chains, but the latest report is that they increased - actually doubled in the past month.
Anyone else have something like this happen? I was told by both my oncologist and the City of Hope that I am not a candidate for a stem cell transplant.

My husband was diagnosed in early Jan of this year with myeloma. Initially they put him on CyBorD (cytoxin, Velcade and dexamethasone). His numbers starting decreasing right away. Then after a month or so some of his numbers started going up again while on treatment. So our oncologist who consults with a specialist at Moffitt Cancer Center decided to change his treatment to Revlimid, Velcade and Dex. His numbers started coming down again, but after 3 cycles his specialist decided they weren't coming down fast enough. So just this week they started him on Kyprolis, Revlimid and Dex. He had 2 weeks off between the changing of the treatments and his numbers increased quite a bit. Praying that this new treatment really does the trick and gets his numbers to come down. We thank God for every new day !

Jerry 410, Are you able to provide a little more information about the kind of myeloma your husband has, e.g, IgA kappa, IgG lambda, etc.? Also what kind of mutations, if any, are involved and is he high risk? What is your husband's age?

My husband is 64 and his multiple myeloma is IGG kappa. His FISH came back all in the normal range.
His Kappa started out at 1549 and is now 399. His IGG started out at 5459 and is now 4942. His
M Spike started out at 3.4 and is now at 2.6 His Kappa went down to 293 and his mspike to 2.2, but with 2 weeks off treatment jumped back up.

The change for me will be adding subcutaneous Velcade to my existing Revlimid/Prednisone regime, and increasing the dosage of the Prednisone

Adding another drug is the usual way that many of the myeloma specialists begin to change treatment since this is often successful. Revlimid, Velcade and Dex is a very common triplet with newly diagnosed myeloma patients.
I restarted treatment after relapse 2 1/2 years ago with Revlimid and Dex. When I restarted my oncologist gave me a bench mark that he wanted me to meet within the first round of treatment. He wanted to see my m-spike drop from over 3.5 g/dl to 2.5 g/dl, or lower. Being an overachiever that I can be, after my first round my m-spike was 1.5 g/dl. But, he had said that if I hadn't reached his goal that he would add Velcade. He continues to tell me that if my m-spike goes above 1.0 g/dl now that he would add Velcade. I've been stable at 0.6 g/dl for over 2 years now.
So, sometimes just adding 1 more drug like Velcade does the trick. If it doesn't then your oncologist will change your regimen to something else. Why has he told you that you aren't a candidate for a stem cell transplant?

Both my oncologist and the City of Hope told me that my Performance Status was too low - about 60%, which precluded a stem cell transplant at this point. That could be reevaluated after four cycles of induction therapy. The failure of Revlimid occurred in my current second cycle.

I start the Velcade (subcutaneous injectable form) next Tuesday, and I hope it puts me back on track.
