32 years old - wondering if lab results indicate myeloma

I started feeling AWFUL -- very awful -- 2 months ago. Found out from routine blood work my thyroid was out of whack. So went to endocrinologist and they noticed my proteins were off and decided to dig deeper.

Here is where the fun begins (i kid, i kid). She didn't think my extreme of symptoms would just be because of the thyroid since it wasn't THAT far off.

Got the first test back and things were pretty out of whack, so she did it again to make sure there wasn't an error before freaking me out or referring me. Now I have been referred to a hematologist and I'm waiting on them at MD Anderson to get back to me on setting up an appointment.

Based on what I've read things don't look all that great from my lab tests...I've put them below and would love to know what I'm in for.

I'm only 32, a mom of 2, full time working and competitive triathlete

Here are some of the results. Original list is much longer, but has loads more of it that she was trying to rule out (rheumatoid arthritis, celiac, hepatitis, etc)

I can see where your doctors could have some concerns. I don't however, know enough about things to give an informed opinion. Hopefully you will get that appointment soon so you can get some peace of mind. There are others here who understand this disease better than me. But I did want to let you know that you have come to a good place to feel understood, and to gain knowledge. And I'll happily keep you and your family in my prayers. Cindy

I just wanted to reply since I was 32 when I was diagnoses. I can say that not many 32 years olds are diagnosed with full blown myeloma! it is rare for your age group!

If I read your labs right, your IGG is high, and you IGA is low, with an M Spike of 1.6. Your total protein is barely out of the range so that's not too bad.

If you look up 'high protein' on Google you get very scary multiple myeloma websites. I know this because I've done it lol. In your case I think it is premature to worry about multiple myeloma just yet! Yes your labs are showing some things that might indicate a disorder such as Myeloma, but to a layperson they are not that much out of range just yet. When I was diagnosed my total protein was about 10X what is should be and my m- spike was about 10 (or 100 since I'm Canadian)

Your Calcium, and your creatinine is in the normal range so is reassuring that your Kidney function seems ok. Did you have a CBC blood test? Was your hemoglobin ok? no signs of anemia?

The hematologist will probably redo the blood work with a bunch of other tests. The other common tests are xrays of all your long bones and maybe a 24 hour urine colection to test it for protein.

I'm so sorry you've been feeling so unwell, missfsu17.

It's very helpful that you've gone through your lab results and summarized them the way that you have. There is one set of results, however, that you haven't included, at least not as far as I can tell, which would be helpful to see. Specifically, did you have any tests done to determine your hemoglobin levels? If so, can you share them with us?

The reason I ask is because determining whether or not someone has multiple myeloma depends on assessing whether they have one or more of the classic "CRAB" symptoms of the disease - high Calcium levels, Renal (kidney) failure, Anemia, and Bone lesions.

Note that it's possible to have elevated protein levels like you have but not have multiple myeloma. In particular, you could have one of the conditions that are often described as myeloma "precursor" diseases -- monoclonal gammopathy of undetermined significance (MGUS), and smoldering myeloma.

These are called "precursor" diseases because it is thought that everyone who is eventually diagnosed with multiple myeloma first had one or both of the precursor diseases.

HOWEVER ... and this is very important ... it's possible to have one of the precursor diseases and to never, ever progress to multiple myeloma. In that sense, it's somewhat misleading for them to be called "precursor" diseases, because not everyone with those diseases eventually develops multiple myeloma.

Here is the full CBC. I think all in normal. I probably am freaking out too just because my mom has a blood cancer (CLL - leukemia). I know its too soon to freak out, but obviously something is going on and looking forward to more answers soon, fingers crossed.

Cheryl G has it right. This could very well be a case of MGUS (or perhaps smoldering myeloma), which may never develop into full blown multiple myleoma and would never require any sort of treatment.

From what I can see, your tests show that you have already eliminated the C, R and A in the "CRAB" criteria, which is great....but a doc really needs to confirm this.

Based on these results, your doctor will likely order a bone marrow biopsy to confirm just what is going on (again, this could very well be MGUS, given the levels of these numbers). This is a pretty simple and quick outpatient procedure to determine just what is happening inside your bone marrow.

He.she will also likely order up some additional tests such as an xray of your skeleton to see if there is any sign of the "B" in CRAB.

There are some great multiple myeloma specialists at MD Anderson. You will be in good hands.

Again, take a deep breath and try not to worry too much. These numbers are pretty low.

Hi missfsu17 - Hi, I agree with the posts and that most likely you'll have more tests.

Over 2.5 years ago, I had a similar situation as you; my endocrinologist found an M spike of 1.5 and referred me to an oncologist. The hardest for me was the 2 month span of not knowing and thinking the worst, since you are waiting for an oncologist appointment and scheduling / taking tests (24 hour urine, scans, and bone marrow aspiration). Eventually I was diagnosed with MGUS (IgG) and am now assessed every 6-8 months with a blood test.

I also understand your concerns with blood disorders - like your mom, my mom also has CLL. Please try to focus on the fact that your numbers are low and you have eliminated 3 of the 4 CRAB criteria.

Thank you all. It is great to know the support. I have an appointment now after doctor review at MD Anderson on 5/5/14. Glad to know they will accept me as a patient to do a consult that day and further testing.

Again thank you for the kind words. Going through these tests, scares, unknowns is very hard to share with friends/family in concerns of worrying them or them having no clue the anxiety it causes.
