Do I have a monoclonal immunoglobulin?

Hi every one.

Would you please help me with this? I am a 28 years old girl. Due to low platelet I've done SPEP and IFE tests. My results are:

Total Protein=72 G/L (60-80)
Alpha 1 Glob.= 1.8 G/L (1-4)
Alpha 2 Glob.= 7 G/L (4-7)
Beta 1 Globulin=5.8 G/L (4.6-8.1)
Beta 2 Globulin=6.2 G/L (1.8-5) HIGH
Gamma Globulin=11.4 G/L (7-12)

SPEP comments: Normal value of gamma globulin (11.4) with discrete zone of a moderate migration restriction with no evident spike.

IFE test : discrete zone of a moderate migration restriction with no evidence of monoclonal immunoglobulin as well as a possible very small IgG kappa included in the beta2 globulin, to be controled in further test.

My hematologist is not concerned at all, and told me to continue checking platelet level (90 000) presuming it is an idiopathic purpura thrombopenic and if this gets lower we can do a bone marrow biopsy.

But when I went home and looked in the internet I was shocked with how serious this can be?

So do I have - yes or no - a monoclonal immunoglobulin? Is it normal to not do any other test?

In addition to a complete blood count (CBC) with differential and a peripheral smear, I would suggest checking free light chains, calcium and creatinine levels.

Doctor, only light chain immunoglobulin testing wasn't t performed. I will ask for it. All other tests were normal, here are the main values :

Calcium 2.25 (2.25 - 2.60)
Creatinine 64 (44 - 80)
Creatinine clearance 94.9 ml/min/1.73 m2

Blood count
WBC 7.9 (4-10)
RBC 4.7 (4-5.4)
Hg 13.2 (12-17)
Reticulocyte count 1.07 %
All WBC subgroups are normal .
Platelet 90000 Low
Platelet mean volume 12.6 (6.5 - 12)

Comment: smear blood controlled, no abnormalities
