Do my blood test results point to multiple myeloma?

by Mountaineer on Thu Nov 24, 2016 2:33 pm

In the last two weeks, I have received the following results for:

LCO IMMUNOGLOBULIN A, QN, SERUM

First result was 1051 mg/dl (normal range is 90-386 mg/dl).

Two weeks later, it went up to 1119 mg/dl.

After these tests, my doctor ordered the following four tests:

Test# 164863 ANA w/Reflex if Positive
Test# 123041 PE (Rfx IFE+FLC), S
Test# 164914 CCP Antibodies IgG/IgA
Test# 006502 Rheumatoid Arthritis Factor

All of these tests came back negative for lupus or rheumatoid arthritis.

It is Thanksgiving today, so I can't go back in to see my doctor until Monday, but I am curious if anyone has experience with these type results.

I am 41. My maternal grandmother had myeloma in the 60's and died at the age of 37. My father's brother currently has it, but is in remission, so it does run in the family.

All my other blood tests and levels are great, but I have lost 35 pounds in the last 5 months, and I am naturally a little concerned.

Thank you.

Hi there,

When you listed all of the tests that were done you said that they came back negative for lupus and RA, but have the following test result come back?:

Test# 123041 PE (Rfx IFE+FLC), S

I'm thinking that is indicating a Serum Protein Electrophoresis and, if positive, a serum immunofixation and serum free light chain test.

If in fact that whole series of tests was done and no abnormalities were detected, it would go a long way towards ruling out a monoclonal protein disorder like myeloma and pointing toward a polyclonal cause for your elevated IgA.

However, if I'm reading the test order correctly it looks as though the serum immunofixation and free light chain tests are meant to be 'reflexive,' i.e., performed only if the serum protein electrophoresis came back positive for an M-spike / monoclonal protein.

If the serum protein electrophoresis comes back negative, I might be inclined to push for the other 2 tests to be performed, as some individuals with myeloma have monoclonal protein levels too small to be captured by the serum protein electrophoresis, and more sensitive tests like the serum immunofixation and serum free light chain test might be required to confirm or rule it out.

Hope that helps and best of luck!

I just had my first biopsy/aspiration done on the 22nd. The results are scheduled for January 3rd.

Back in November, during blood work-up with my GP, my IgA levels came back at 1051 mg/dl

Two weeks later, it was up to 1119 mg/dl.

On December 8th, the most recent result was 1224 mg/dl

Additionally, on my IFE 1 result, it showed:

'Immunofixation shows IgA monoclonal protein with lambda light chain
specificity.'

My LCO M-Spike is 0.7.

Lastly, the blood work from the day of biopsy already came back.

In a three week period, I noticed two major differences:

My Lymphocyte count went considerably up, and my neutrophil count dropped considerably.

My WBC count dropped almost in half, as well.

With all this being so sudden, we took the initial referral to the Rocky Mountain Cancer Center in Denver. My doctor does not exactly foster a trust or confidence with his bedside manner. RMCC is not a myeloma-based specialist center, either.

We are anxious for the results and plan on seeking another doctor as the journey continues.

Any advice would be greatly appreciated.

Merry Christmas and thanks for the great wealth of info on here!
