what do you think is more effective?
dex dosage all in one day or spread out a couple of days

Hey Al,
I'm not sure I understand your question. During his initial therapy, EJ was taking dex four days a week. About halfway thru his treatment, the doc knocked it back to twice a week. The dex accomplished what it was supposed to.

if you are taking 20mgs. a week,is it more effective taking it on let`s say a Monday or 12mgs on Monday and 8mgs. on Tuesday

I take it IV on Tuesday and Wednesday for three weeks then orally for one week. 20 mg each dose.

When Joe was on it and now he is off because his labs are good. Now only on Zometa IV once a month. He had to take 40 mg of Dex every Friday in one dose (awful for two to three days) till it wears off). three days of normal??? then time to take it again!! Now that he is off the Revlimid he is off the Dex. Thank God.
Will one of you tell me EXACTLY the lab listing that shows the M level or spike, Is that the same as a tumor marker?? Please...PLUS would like to have some input from someone who DID NOT and CAN NOT have a stem cell transplant! NONE of the multiple myeloma orginazations EVER discuss that group of patients. WHY...Thanks, Dotty

I don't have a good answer to your questions...When they dropped EJ's dose down they told us it was because they were finding that you didn't need to take such a high dose to get a response. But I don't know about dividing up a dose and spreading it over several days.

On EJ's lab reports his M-spike is reported as "paraprotien."
Regarding SCT - there are many people on the site who either elect not to have a SCT, or are ineligible. There has been much debate over the need to have a SCT given some of the novel therapies availalbe (Velcade, Revlimid, etc). Even if Joe can't have a SCT, I think the process is still pretty much the same - induction, consoldiation and maintenance. A friend of mine's mother had multiple myeloma and she was not able to have a SCT. I believe she was treated with melphalan as part of her initial therapy, along with Revlimid and dex, and had has been in CR for almost two years.

My initial therapy was Velcade and Dex. I took 40 mg of Dex with each treatment of Velcade, all at once, with no problems, except for eating everything in the fridge.

My husband is on maintenance following two stem cell transplants and takes a 21-day cycle of Revlimid with weekly Velcade and 8 mg of Dex. He achieved CR during the transplant process.

Ideally dose is given all on one day. If you / your doctor are trying to minimize side effects, then consecutive day dosing could be done.
