Now that daratumumab (Darzalex) has been approved for relapsed myeloma, I am wondering if off label use is possible for smoldering multiple myeloma or untreated myeloma, such that a person could personally pay for the medication and be under a specialist's care for administration.
How much would this cost?

There is a clinical trial testing different doses of daratumumab in smoldering multiple myeloma patients. You can find information about it here:

Near the page at that link is a link which, if you click on it, will show you locations where the trial is available.
If you're willing to pay for the daratumumab yourself, you might be able to get a specialist to prescribe you the drug for smoldering myeloma outside the trial setting, but they probably would recommend participating in the trial as the first option.

The cost per month depends on how frequently the drug is dosed and the patient's weight.
