When is multiple myeloma treated with Cytoxan?

I had Cytoxan (cyclo­phos­phamide) as part of my treatment for breast cancer, and I see here where it is given for myeloma as well.

Is it usually given as first treatment, or only if other treatments do not work?

I also see where it is give for stem cell transplant as well.

Cytoxan is more likely to be used in Canada, New Zealand, Australia and some Western European countries as a first line treatment of myeloma as part of the CyBorD regimen (cyclophosphamide, Velcade, and dexamethasone).

In the United States, Revlimid is more likely to be used in combination with Velcade (bortezomib) and dexamethasone (RVD, or VRD).
