Am I in complete remission (CR)?

My M spike came down to 1.2 g/dL (12 g/L) after five cycle from 3.9 g/dL (39 g/L) after fifth cycle of Revlimid plus dex. After sixth cycle B2 raised but no M spike. After seventh cycle B2 raised but no M spike. My physician ordered immunoelectrophoresis and it says myeloma band detected, kappa light chain detected, IgG component detected. Suggestion of monoclonal gammopathy - positive for IgG and kappa isotopes.

Kappa free light chain serum 27.1 from high of 378, k/ l ratio normal of 1.49(first time since treatment), lambda normal at 18.1,

Am I in complete remission (CR)?

Hi,

At this point, given that you still are positive for a monoclonal protein on your serum immunofixation, you haven't reached a complete response. Technically, you'd also have to have a bone marrow biopsy to establish whether you have a complete response, because having less than 5 percent plasma cells on that biopsy is part of the definition of complete response.

Even though you haven't yet achieved a complete response, it seems like you are on the way, so you may eventually achieve one.

Thank you, Terry H. I had three bone marrow biopsies. My high before starting Rev plus dex was 40 to 50 percent. After five cycles, in September, bone marrow biopsy is five to ten percent.
