Celgene Patient Support® is pleased to announce a change to the Celgene Commercial Co-pay Program.
In 2013, patients on oral Celgene products who enroll in the Celgene Commercial Co-pay Program will have a co-pay responsibility of $25 per prescription. This change reduces patients' co-pay responsibility from the previous level of $100, providing an additional benefit to eligible patients of $75 per prescription.
To qualify for the program, patients must meet the following eligibility requirements:
- Patients with federally subsidized health insurance are not eligible; the program is intended
for patients with commercial insurance coverage
- Residence in the United States or Puerto Rico
- Annual household income of $100,000 or less
The Celgene Commercial Co-pay Program is designed to offer patients with commercial insurance an option for co-pay assistance. Patients are enrolled by Specialty Pharmacies, who verbally communicate the benefits and requirements of the Celgene sponsored program. Once patients are approved for the program, a letter will be sent to the patient and the patient’s physician notifying them of the approval.
Once approved, patients will be charged $25 per prescription to cover their co-pay responsibility up to a maximum benefit of $10,000. If the patient does not qualify for the Celgene Commercial Co-pay Program or if the $10,000 benefit is exhausted, the patient will be referred to Celgene Patient Support.
