Brand new to all of this

Hi All~ I'm brand new to all of this & still waiting for the proper read on all my labs. I did test initially with high kappa & high kappa lambda ratio (serum) which warranted a 24 hour urine study. It only show albumin of 94. mg/24 hour with a normal ratio of 30-150. Yet I'm reading that urine shouldnt contain protein elsewhere. It's also stating it's 100% Albumin of the 4.7mg/dL.

Can anyone tell me if this constitutes further concern?

Secondly, my recent bone scan showed significant progression of osteopenia in my femur. It's progressed across the board, but the femur more so. It is almost osteoporic. I'm early onset osteopenia which was first Dx at 47. Again, I'm eager to get to the bottom of this & the process so far has taken some months.

Lastly, can anyone recommend a good hematologist in NC? The other concern I have going on is Mast Cell Activation Disorder & Ehlers-Danlos. Someone able to assist with all of this in mind would be perfect.

Hi There,

Initially, you are really going to want to get all of the following lab tests done if there is a suspicion of multiple myeloma. Having a high freelite ratio is a red flag. You likely had several of these other lab tests already done if you know your freelite ratio...it would be kind of odd for your doc to initially order only a freelite assay and a comprehensive metabolic panel, and not a SPEP, IFE, CBC, immunoglobulin tests, etc.

A 24 hour UPEP is a good, next step, but the other lab tests should also still be done to get a more complete picture, if they haven't been ordered already.

If the lab tests indicate multiple myeloma, you then are likely going to want to get a bone marrow biopsy to see what what is going on with your plasma cells in your bone (also see the link above). And given your DEXA scan results and the other items you mentioned, you are going to want to get an imaging study such as a skeletal xray survey or PET/CT (depending on the results of your blood and urine tests) to see if there are any lytic lesions present (again, see the above link).
