I know that the main thing to be watching is the plasma cell percentage, which for me is at 8%. But what I don't know is the significance of the "Slightly hypercellular marrow (60%) with trilineage hematopoiesis".
And also bottom of report included " Special Stains: Iron stain reveals the presence of rare erythroid iron incorporation." What does this mean?
Below is what the report said for diagnosis:
Peripheral blood, bone marrow aspirate, touch imprints, clot and core biopsy:
- Slightly hypercellular marrow (60%) with trilineage hematopoiesis
- Plasma cell neoplasm, with approximately 8% plasma cells
- Minute monoclonal plasma cells identified by flow cytometry
- Plasma cell immunophenotype: bright CD38+ / Lambda+ / CD56+
- Lambda-restricted plasma cells identified by in situ hybridization
- No evidence of a monoclonal B-cell population
- No evidence of amyloid deposition
- chromosome analysis pending
Comment:
The bone marrow is slightly hypercellular (60%) with trilineage hematopoiesis. Plasma cells are variable, and are estimated to comprise from 5-10% of total cellularity by CD138 immunostaining. By flow cytometry and in situ hybridization staining, a monoclonal lambda-restricted, CD56 positive plasma cell population is identified. There are also occasional polytypic plasma cells identified by flow cytometry and in situ hybridization. CD56 immunostain is also performed to attempt quantification of neoplastic plasma cells. Overall, neoplastic plasma cells are estimated at approximately 8% of marrow cellularity. There is no morphologic or immunophenotypic evidence of a monoclonal B-cell population identified. These findings are those of a plasma cell neoplasm. Correlation with clinical, radiographic and laboratory findings is needed.

I got these back in June but the only thing I knew to ask the doctor at that time was my % of marrow. I'm now finally really looking at my results and have these questions.
