Bone marrow biopsy results questions

by Toni on Fri Aug 29, 2014 6:04 pm

Hello all,

The BMB confirmed my MGUS diagnosis, for which I am grateful.

Before I go into internet search h@ll  Can someone help me understand some statements here?

1) "The erythroid elements show mild megaloblastoid."

2) "The myleomonocytic elements show mild shift toward immaturity."

3) "There are increased plasma cells, mainly scattered singly in an interstitial distribution"

Note: The comments state "Abnormal plasma cells comprise 5-10% of the cellularity." However in the detailed analysis it states, "Bone marrow differential ... Plasma cells: 2.0%".

4) "A Prussian blue-stained aspirate smear demonstrates absent storage iron and decreased but present sideroblastic iron."

The only thing besides the MGUS diagnosis I didn't know is that I should probably take iron supplement because of absent iron stores.

Your input would be greatly appreciated!
Thank you :-)

Did your doctor explain any of this to you ? I would ask your doctor to take the time and go over every item. Its his job to explain this to you until you understand it. Sorry, but I think you need to ask him. Good luck, i know this is not helping you with your questions.

I did and he explained it to my satisfaction.

My questions are simply things that I did not understand from the written report because when I came home I was able to read the report at my leisure.

I just can't figure some of the wording even with my usual internet searches so I hoped to get some answers here :-)

Just to clarify a bit ... I'm primarily interested in the two different percentages cited.

"Abnormal plasma cells comprise 5-10% of the marrow cellularity." However in the detailed analysis it states, "Bone marrow differential ... Plasma cells: 2.0%".

I believe the 5-10% was "There are increased plasma cells, mainly scattered singly in an interstitial distribution"

So when you quote what your % plasma cells are, how is it quoted?

I'm no microbiologist, so when it says the 5-10% is scattered singly in an interstitial distribution, the sense I got was it was a cluster of cells separate from the rest.

See how I'm making myself crazy trying to get a PhD in pathology in one weekend? :P

I'm also putting off doing housecleaning ...

"Morphologic characteristics: Distribution: interstitial (clusters not associated with blood vessels), or sheets"

Maybe this is my answer. It's simply how it's distributed in the marrow?
