Greetings all...
I have a concern as a result of blood in my urine. I was diagnosed IgA lambda multiple myeloma in July, 2013. Had Revlimid / dex induction in the fall of 2013, followed by autologous stem cell transplant (ASCT) at Mayo Rochester in February 2014. I am VGPR, and on 10mg Revlimid maintenance for the past 14 months or so. Feeling good overall!.
Eight months or so ago I had visible blood in my urine, which soon went away. My local onc chalked it up to Xarelto (rivaroxaban), which I take due to a DVT in July, 2014. But a couple of months ago, I finally got around to a general physical, which showed blood in urine on dipstick test. Followup with urologist since that physical showed the following:
Sonogram of kidney: clear
Scope of urethra and bladder: clear.
But the dipstick urine test a couple of days ago again showed "a lot of blood", but not visible blood. The urologist said "good news - no protein in urine". But I didn't think to ask him which protein he was talking about. And he ordered the CT because he is concerned about all this ...
I am scheduled for CT of kidney in a week.
Anybody had history with blood in urine?
I know that foamy urine is usually caused by protein, and can indicate relapse. I have foamy urine (my IgA has been in the 200 range for months - which I believe is quite good). My M-spike runs at 0.1 g/dL or so, but I recognize that M-spike is not particularly sensitive for those of us who are IgA.
Is IgA nephropathy a possibility here? Are folks with IgA myeloma at particular risk for IgA nephropathy / hematuria?
Anything else I need to consider here?
Thanks
Wesley

If you haven't done so already, you should contact your oncologist at Mayo with your questions. Do you also have a local oncologist? If so, start there.

Re: Blood in urine - concerned
Wes,
I went through a nearly identical sequence of tests with my urologist a few years ago (before I was diagnosed with SMM) when a routine physical showed I had hematuria (blood in the urine).
After all the tests, everyone finally agreed that the cause of the hematuria was idiopathic (which I was told is not that uncommon of a diagnosis with cases of hematuria). After being tested again after a couple of annual physicals, the hematuria just disappeared.
I remember being freaked out at the time and jumping to premature conclusions about kidney or bladder cancer and was happy to get all the tests behind me to rule out those possibilities.
But given you are also on multiple myeloma drugs, I think Nancy's suggestion also makes a ton of sense.
Let us know how things turn out.

Re: Blood in urine - concerned
My husband was just diagnosed with stage 2 myeloma and one of his symptoms was blood in his urine. We did find that he has a few kidney stones, and they can rough things up a bit on the inside. That's just an idea.
