Biopsy and RNM total body
I have done 2 cycles of VRD , and before to proceed with further cycles ,with the doctor we have decided to re-evaluate the multiple myeloma , and so I I have done a biopsy and a RNM total body. The results is getting me crazy, biopsy is very positive only 2% of plasmacells versus >15% in the previous one, while RMN is showing new bone deseases. Now I have done a new biopsy and new Rx to the bone deseases and I am waiting for the results. I have a non secernent multiple myeloma difficult to monitor , free lite chain are in the range since the beggining of VRD cycles. Any similar exeperience?

Hello !
Overall if the amount of myeloma in the bone marrow is falling that is the most important measure of response. If the bone disease seems to be worse it would be hard to attribute it to myeloma if the bone marrow results are so much better. If you did not have an MRI when you were diagnosed (most of the time we start with a series of xrays called a "skeletal survey")then it will be much harder to say that you have "new" bone disease with an MRI performed months later. Apples must be compared with apples. I feel good that the marrow is demonstrating a nice response and you should as well.

Thanks for your reply. I have the previous MRI and new bone diseases have been detected comparing the two MRIs.

It is hard to understand why your cancer would be disappearing but the bone disease which is secondary to the cancer is getting worse. One possiblility would be that you suffered such significant osteopenia (bone thinning) before you were diagnosed and treated that the bone collapse and fractures are continuing. Because I am so limited in terms of not having access to the entire picture of your disease I must defer to your myeloma doc for the final answer.
