Biaxin with Pomalyst and dexamethasone

A friend of mine has multiple myeloma and is refractory. He was using Pomalyst and dex with no improvement. His oncologist tried adding Biaxin [clarithromycin] to the mix and had good results. Does anyone have more information on this or experience with the combination?

Thanks for any thoughts you can share.

Hi Dale,

The combination of Biaxin (clarithromycin), Pomalyst, and dexamethasone is sometimes abbreviated "ClaPd". Results of a trial studying it were presented at last year's American Society of Hematology annual meeting. They are discussed in this article,

(look for the section with the heading "Pomalyst-Clarithromycin-Dexamethasone")

The response rate seen with the combination is quite good for patients who have been as heavily pretreated as the patients in this study. (They had a median of five prior therapies.)

Some physicians combine Biaxin with Revlimid and dexamethasone to seek better response rates than might be achieved with just Revlimid and dex, and Pomalyst is chemically similar to Revlimid (it's in the same class of drugs), so it was natural to try it with that drug as well.

I have seen articles in the Beacon about Biaxin for myeloma treatment, but have not heard about anyone using it or how it was being used with other drugs. Please keep us up to date with how he does and any specifics you can add about his prior treatments, lab results, staging, etc.

Biaxin is cheap and readily available so if it works well I am sure a lot of people will be interested in it.

They tried me on Pomalyst. I had severe stomach problems, had to be hospitalized. Can't take any chemo on empty stomach, had stomach cancer with 75% stomach removed on top of multiple myeloma.

Tried Biaxin with Revlimid, that didn't work for multiple myeloma. Biaxin also affected stomach and tore colon up.
Now trying Cytoxan [cyclophosphamide] with 20 mg dexamethasone. Will know by end of month if that's working.
