Hi everyone,
I am getting some mixed messages about NSAIDs safety for those with smoldering myeloma.
I realize NSAIDS are contraindicated for those with active multiple myeloma due to potential Kidney damage. Is this also applicable to smoldering myeloma as a preventative measure or can we take them in moderation provided we have normal kidney function via normal creatinine, eGFR.
Are creatinine and eGFR adequate tests to assess and monitor kidney function?

When I was diagnosed with MGUS/smoldering both oncologists that I saw told me to stop using NSAIDS and to use acetominophen if I needed something for pain. That was hard since I had significant arthritis pain in my knees. A year later when I developed elbow pain that seemed like tendinitis my oncologist said that I could take Ibuprofen on an emergency basis if acetominophen wasn't helping. It turned out that I had a fractured arm at the site of bone lesions that had developed and I had progressed to active multiple myeloma.
So, I think that it is better to avoid NSAIDS if at all possible because you never know when, and if, you will progress to active multiple myeloma.

Hello Dana,
I do not think that taking NSAIDS is an absolute no-no. Yes they should be taken in moderation. If patients have baseline kidney dysfunction they are contraindicated though. The creatinine and GFR are good ways to follow kidney function in general. Without question NSAIDS can damage the kidney and compromise kidney function.

An alternative is Celebrex which my oncologist has prescribed.

Two of my specialists told me to not just say "no" to NSAIDs, but to be careful and judicious with them as a SMM patient. So, I think a bit before I pop any ibuprofren (my NSAID of choice, as I find naproxen to be worthless and I only use baby aspirin as a prophylactic for heart disease). If I have a headache, I try drinking some extra water or tea and/or relax. If I have other kinds of pain, I question whether I really need the NSAID or if I should instead just tough it through the pain. And if I do need to take an ibuprofen, I limit the dose.
