Living in Europe it seems that some doctors in the US start to rethink the traditional approach of having an early stem cell transplant right away with new drugs such as Revlimid, Velcade and Carfilzomib available.
Which institutions / research centers generally do not favor to proceed with an autologous stem cell transplant (for eligible patients) after an initial induction therapy?
How do they treat patients instead (without an autologous stem cell transplant)? What kind of schemes do they apply for long-term medication then? What are the doses?
Who was offered to proceed with an autologous stem cell transplant after an induction therapy but decided to postpone it receiving other medication meanwhile with the option of having an autologous stem cell transplant at some time in the future?

I was diagnosed for multiple myeloma 5 years ago and 3 years ago started my therapy (VD and then VTD).
Though all my doctors suggested ASCT (I'm 50 years old and in good phisical codition) I decided, following intesive search and articals readings not to go for ASCT now since it didnt demonstrated any clear OS and TTP advantage over novel therapy. I'm now 1.5 years in remission, feeling fine, working and doing a lot of sport.
For sure any one of us is a different case but because of that I think that this decision is not obvious and one have to decide based on his specific feeling, preferences and phiscal condition. Only the future (hopefully long future) will tell if I was right in my decision! but this is true for any course of treatment.
