Above normal serum kappa free light chain results

I have IgG lambda myeloma. Following a stem cell transplant at the end of January, I noticed that for the past 4 months my serum kappa free light chain level has been above normal for the first time. Not wildly above normal, but consistently in the 2.6-2.9 range, with top normal being 1.94. Lambda and ratio are normal.

Any conclusions to be drawn from this?

From my understanding, the kappa/lambda free light chain ratio is the most important. However, if your kappa free light chain goes haywire, then I would worry. I would suggest you ask your onc at your next visit.

While a normal free light chain ratio does not suggest a monoclonal process, it would be useful to check other monoclonal protein studies as well, including SPEP, UPEP, serum and 24-hour urine immunofixation.

Sometimes, during immune reconstitution after a stem cell transplantation, the uninvolved free light chain can paradoxically go up. A phenomenon, known as ’secondary monoclonal gam­mop­athy of undetermined significance’, is occasionally encountered in the post-transplant setting, and could be associated with a favorable outcome.

Ok Dr dont take this the wrong way you are a DR so i am sure you are very smart but you should try to talk on a lower lever to us people so we can understand what you are saying thank you Joe

Hi Joe,

I'm sorry that you found it difficult to understand what Dr. Kapoor posted. If there are words or phrases that he used that you don't understand, please let us know, and I'm sure someone here in the forum can help you out. There are so many helpful forum members, including expert doctors, who should be able to help.

I also highly recommend coming and reading the postings here in the forum regularly. That's what I started doing a couple of years ago, and I've learned so much along the way. There may be discussions and words that you don't understand at first. Over time, though, you'll find yourself understanding more and more. And, like I said, you can always ask questions about things you don't understand.
