100 day results - still a little confusing

My husband had his 100-day exams/tests.

His creatinine level decreased from 6.7 to 3.5

He was 50% involved at diagnosis, he is negative now (no sign)

His KLC ratio in blood is now normal

His KLC ratio in urine is 3.53 (it actually went UP)

He has an impaired kidney since all of this, but is not on dialysis.

I am confused as to why the bone marrow biopsy came back negative (no sign of cancer) and the urine is out of control.

Any ideas?

Hi LadyLib,

Well, a couple of things occur to me as I read this.

First, is his disease light-chain restricted?

If not, what did his Serum IFX and SPEP results tell you? Did they detect any monoclonal proteins to corroborate the BMB results?

UPEP results can be influenced by renal function so that may be why you see this discrepancy between the UPEP and serum freelite assay K/L ratio results?

It seems like a lot of things are improving, including his kidneys and plasma cell involvement....so congrats!

The more I learn, the more I feel like I have so much more to learn:)

I have to do some research with the doctor to understand the urine results.

We left a message for them to call tomorrow. So, hopefully we will have an update.

As indicated above, I am trying to understand the post transplant results for my husband at Day 100.

Bone Marrow Biopsy
At Diagnosis - 50%+ involved
Day 100 - Negative (no sign)

Kidney (Creatine Level)
At Diagnosis - 6.7
Day 100 - 3.5

BLOOD RESULTS
At Transplant - IGA Quantative 53
Day 100 - IGA 37 (Normal: 63-484) - LOW

At Transplant - IGG Quantatitive 434
Day 100 - IGG 413 (Normal: 540-1822) - LOW

At Transplant - KLC Ration 2.51
Day 100 KLC Ratio - NORMAL (.96) NORMAL RANGE .26-1.91

URINE RESULTS - THIS IS WHERE I AM WORRIED????
At Transplant - KLC 2.95
Day 100 - KLC 3.53 (Normal: .14 - 2.42)

At Transplant KLC Ratio - 18.4
Day 100 KLC Ratio 12.1 (Normal: 2-10.4)

I cannot understand why the urine seems so out of whack..We are waiting on more info on the urine this week.

My doc said not to worry about the urine if the serum is normal. I believe that one of the doctors who posts here said the same thing recently.

Our doctor said the same thing. My husband's uninvolved light chain was elevated, making the ratio abnormal. The doctor on here said not to worry either -- it was not of importance.
