For the 4th month in a row, now 8 months post transplant, my M-spike is holding at 0.1 g/dL (1 g/L).
For context, I was at 0.6 g/dL pre-transplant, 0.3 g/dL 90 days post transplant, 0.2 g/dL 120 days post transplant, and 0.1 g/dL the last 4 months.
I have faith in my doctors saying that stability is great and we don't need to reach zero to be pleased with treatment. That said, I also have read that "complete response" is statistically more indicative of a longer remission, especially for "high risk" patients.
Instead of once again asking my doctors if I should be worried about the lingering M-spike, I've decided to turn to my friends on the Beacon forum to help me understand and work through this. I'm currently receiving maintenance of Kyprolis, Pomalyst, and dex (KPD) (half dose from my initial induction), and the plan is to stay with this as long as three years, assuming no relapse or side-effects.
Should I be concerned? Looking at other treatment? Pressing my doctors more?

Based solely on what happened with me, I'd say that you could just hang in there and see how that number goes. I was still at 0.3 g/dl at 9 months post-transplant. It was over a year before my M-spike dropped to 0 and even longer before I had a negative immunofixation. I've not had a detectible M-spike or positive immunofixation for a couple of years now. (Knock on wood and everything else in the room.)

Hi Mark,
EJ had an M-spike of 0.1 g/dl (1 g/l) after his stem cell transplant and remained in remission without maintenance for 54 months. The fact that your M-spike continued to go down after the 100 day mark is a good sign.
