Interpreting FISH results

Looking for help with a FISH report. I am SMM last FISH showed 1q21 gain.

New FISH shows abnormal PPC multiple myeloma FISH. Rearrangement of IGH in 53% of cells. Repeat may be needed to identify the IGH (14q32) partner.

No other mention of any other abnormalities no mention of 1q.

Aany help would be appreciated.

Hi Vicki,

I think you probably know that a +1q21 ("1q21 gain") is an indicator of a less than favorable prognosis with respect to the progression of the disease and overall survival.

A translocation ("rearrangement") of 14q32 also is a less than favorable prognostic indicator.

You can easily google on either "1q21" or "14q32" and "multiple myleoma" to find out the results of various studies on these specific gains and translocations with respect to multiple myeloma.

Suggest you talk it all over with your doc. I find that FISH writeups are very hard for the average patient to interpret, including myself.

Thank you for your reply..My first BMB was 3 years ago I was MGUS 5.5% With 1q21...Now Second BMB shows 10-15% (SMM) with a FISH of IGH 53% but without the 1q previously shown..

My question was just regarding the FISH the IGH 53%..the partner was not identified advising possible repeat to help identify on second FISH..no showing of the original 1q and the first only showed 1q very different results 3 years apart..Thanks..

That's interesting that the 1q gain disappeared. That's great news. As you likely know, one's genes and plasma cells can continue to mutate and change with this disease over time. Based on my readings, I thought it was more typical that when a person develops new genetic mutations that they unfortunately usually still hold on to the earlier mutations.

I'm also not clear that knowing the 14q32 partner in the translocation is key to understanding the importance of this mutation with respect to your multiple myeloma prognosis.

Thank you Dana..Yes there is so much written on the progression of SMM due to the bmb/FISH result I would like to get insight to the IGH translocation (BA) at 53% if the partner was not found..
