Dear Dr. Berenson,
My doctor has treated me with six cycles of Velcade-dex on a 1,4,8,11 every 21 days schedule. Now he wants to go to a maintenance schedule. What might a Velcade-based maintenance schedule look like?

Patients in our practice do undergo a maintenance therapy. If patients had been on dexamethasone, we also continue steroids.
Generally, we give Velcade at a 1.3 mg/m2 every other week and dexamethasone at 40 mg IV every other week.
We also like to include oral Medrol (methylpredinsolone) at 20 mg every other day between the dexamethasone dosing.
We think this is the optimal way to continue maintenance. However, there is no randomized data to show that this actually is better than NOT giving you maintenance treatment.
I would add that I was the first author on the initial study that showed that steroid maintenance worked back about 8 years ago. Thus, I am a believer in maintenance therapy.
