Hi everyone.
I have IgG lambda MGUS. I've been stable with a small M-spike (0.3 to 0.5 g/dL) for several years. My last blood draw was the same, thankfully, but my quantitative IgM level was 48. Over the past 6 years, it has hovered in the 60s, maybe 70s, but never below. This level of 48 is within normal range for my lab.
Is the lowered level of IgM a reason to be concerned?
I am concerned because I haven't seen this low in 6 years and am worried about immunoparesis. Can IgM levels fluctuate this much?
My IgA is normal and is where it usually lies in the 200s. My IgG is 1595, which is on the high side of normal, but I have been that high over the years. M-spike is 0.4 g/dL, free light chain levels and ratio are normal.

Hi Winter12,
Welcome to the forum. I wouldn't necessarily jump to the conclusion that you are in the early stages of developing immunoparesis. I would instead wait to get a couple more test results under your belt before you and your doctor draw any conclusions. Like my specialist likes to say, "If you don't like your test results, just wait and get re-tested"

Immunoparesis (which you don't appear to have) isn't necessarily something to be over concerned about. My IgG and IgM are well below minimum levels, IgM has been around 14 (0.14 g/L) for the last three years, and I haven't suffered from anything other than the usual (occasional) infections most people get.

I think I have immunoparesis with an IgG of 2780, an IgA of <6 (actually 1.23), and an IgM of 17. My hematologist / oncologist doesn't seem to be alarmed. My kappa / lambda ratio is normal, and I have 2-4% plasma cells from bone marrow biopsy.
I feel great. I am blessed that I don't get ill. It is amazing. Hope this helps.
