Rise in polyclonal immunoglobulin: does it mean anything?

by Mdg on Tue Sep 25, 2018 10:14 am

So I just received my blood work results from latest monitoring. To recap I was diagnosed with IgM MGUS in 2016. For the most part, all my blood work has been in range and consistent for last 2 years. This includes CRAB, free light chains, free light chain ratio, kidney function, etc. My IgG has consistently run between 540 mg/dL and 600 mg/dL, which is slightly on the low side.

However, the one value that changed significantly in the last 6 months is my IgM magnitude. For the most part my IgM magnitude was averaging 610 mg/dL for the last 2 years. It now registers 975 mg/dL.

Also, I might note that My m-spike stayed constant at 0.5 g/dL or 500 mg/dL.

Correct me if I’m wrong based on my understanding of the formula that's been posted numerous times here in the forum, my

Involved polyclonal level September 2018 = total level - M-spike
Involved polyclonal level September 2018 = 975 mg/ dL - 500 mg/ dL = 475 mg/ dL

Whereas, prior to September 2018, my average involved polyclonal level was 610 mg/dL - 500 mg/dL = 110 mg/ dL.

So, assuming I understood the formula correctly, my polyclonal IgM, became elevated. Is this something to be concerned about? Is it representative of disease progression?
