Enchondroma to Plasmacytoma. Possible??

by Live4Health on Sat May 19, 2012 6:58 pm

I recently had xrays done for my Rheumatologist, and the results showed that I had a couple of lucent defects in my left knee. One in the tibia, and one in the femur. The one in the tibia was found in 2006, after a fracture, and was determined to be an enchondroma. It has been monitored since then, but this time the xrays showed changes in that one (growth and no popcorn appearence), in addition to the new one in the femur. The differential diagnosis was the usual "fog machine" minus a few possibilities, but included plasmacytoma which is not part of the fog machine. It was my understanding that progression for an enchondroma was generally low grade chondrosarcoma. Is it possible that plasmacytoma was included as a safety net because I have MGUS, or is it common for enchondroma to progress to plasmacytoma?

I'm waiting for the MRI referal, but my mind is going a million mile a minute. I haven't been able to find any info on such a progression. Please point me in the right direction.

Thanks in advance. :?:

Anyone have any ideas about this. I would really appreciate any thoughts. I'm still waiting for the MRI referral, but would like to have some have some feedback or ideas.

Hi Live4Health,

Is it possible that the enchondroma did not progress to plasmacytoma, as you suggest...and that rather you have a plasmacytoma where you also had enchondroma?

Did any of the doctors use the word "progression" or is that what you speculated based on your knee history?

Perhaps, these are not related events .rather they could be 2 separate unrelated events?

I agree with your first thoughts. I think that plasmacytoma was included becaused of your history of MGUS and the fact that two lesions were noted. Has there been a sudden incresase in your M-spike or other signs of MGUS progression (anemia, rises in either calcium or creatinine)? Endocondroma would not evolve or transform into plasmacytomas. We will need to await the results of the MRI and potential biopsy of the lesion if it remains questionable after the imaging study.
Tibial myelomatous lesions are rare (but can happen).
Good Luck and please keep us informed.
